Patient Information and Consent Forms
 
If you already have an appointment schedule, you can submit patient information and/or consent forms to us using the options below. You do not need to submit all the forms on this page, only the one applicable for your appointment. Hospital staff will tell you what is required when you book your appointment. Please note: submitting one of the forms below will not schedule an appointment for you.
 
Veterinarians are not permitted to display prices on the internet. You will need to contact the clinic for an estimate.
 
Some forms are available as pdf files. You will need need a pdf viewer such as Adobe Acrobat to view the file. You can fill in the file and add an electronic signature, then email the file to us at acres@ns.aliantzinc.ca
 
For consent forms, completed pdf files are preferred. However, during the COVID-19 pandemic, consent are also available as online forms.
