Patient Information and Consent Forms

If you already have an appointment schedule, you can submit patient information and/or consent forms to us using the options below. You do not need to submit all the forms on this page, only the one applicable for your appointment. Hospital staff will tell you what is required when you book your appointment. Please note: submitting one of the forms below will not schedule an appointment for you.

Veterinarians are not permitted to display prices on the internet. You will need to contact the clinic for an estimate.

Some forms are available as pdf files. You will need need a pdf viewer such as Adobe Acrobat to view the file. You can fill in the file and add an electronic signature, then email the file to us at [email protected]

For consent forms, completed pdf files are preferred. However, during the COVID-19 pandemic, consent are also available as online forms.

New Client Form

Are you a new client? Please complete this form to provide us with information for your pet’s medical records. This is especially helpful during the current pandemic as submitting your information online will help decrease human-to-human contact.

Surgery & Anesthesia Consent & Admission Form

Please complete this form if you have a surgery appointment scheduled for your pet. Surgeries include: spays, neuters, lumpectomies.

Dentistry & Anesthesia Consent & Admission Form

Please complete this form if you have a dentistry appointment scheduled for your pet. Surgeries include: dental cleaning, tooth extraction.

Euthanasia Consent Form

Please complete this form if you have a euthanasia appointment scheduled.

Medical records can be submitted online using this form. For large file, or for images (such as xrays), you will need to send us a email.
